An internal report released by Rheinmetall Denel Munition (RDM) into an October 2021 fire and "flashover" at the company's Somerset West facilities found that an electrical fault most likely caused the blaze.

The fire occurred at the N86 building on 31 October last year and follows a similar, deadly blast at the same site in 2018 that claimed eight people's lives.

RDM said this week that the investigation into the fire at the N86 building determined that the fire was most likely caused by an electrical fault in a light fixture in the building.
